An Assessment of Nurses’ Role in Managing Patients with Drug-Resistant Tuberculosis in Yobe State

Background of the Study

Drug-resistant tuberculosis (DR-TB) is a significant global health challenge, particularly in low-resource settings where inadequate treatment regimens and poor adherence to prescribed therapies exacerbate the problem (Maguire et al., 2023). The World Health Organization (WHO) has identified DR-TB as a public health threat, with increasing incidence rates in countries such as Nigeria. In Yobe State, located in northeastern Nigeria, TB prevalence remains high, with a notable rise in cases of drug-resistant strains. Nurses are at the frontline of patient care for TB, including drug-resistant types, playing a crucial role in administering treatments, monitoring patient progress, and providing support and education on drug adherence.

The effective management of DR-TB requires a comprehensive approach, including proper diagnosis, timely initiation of second-line drugs, and consistent monitoring for adverse effects and drug resistance patterns. However, challenges such as limited access to second-line drugs, insufficient training, and the stigma associated with TB can hinder the ability of nurses to effectively manage DR-TB patients in Yobe State. This study aims to assess the role of nurses in managing drug-resistant tuberculosis, focusing on their involvement in diagnosis, treatment, patient education, and overcoming the obstacles they face.

Statement of the Problem

In Yobe State, managing drug-resistant tuberculosis (DR-TB) is complicated by several factors, including inadequate healthcare infrastructure, insufficient training for healthcare workers, and social stigma. Nurses, being the primary caregivers, often face the challenge of balancing the technical aspects of care with the psychological support required by patients. Despite their critical role, the effectiveness of nursing interventions in managing DR-TB has not been adequately assessed, leading to gaps in care delivery. This study seeks to evaluate the extent to which nurses in Yobe State contribute to managing DR-TB, the challenges they encounter, and the strategies required to enhance care.

Objectives of the Study

1. To assess the role of nurses in the diagnosis, treatment, and monitoring of patients with drug-resistant tuberculosis in Yobe State.

2. To evaluate the challenges nurses face in managing drug-resistant tuberculosis in the region.

3. To propose recommendations for improving the management of DR-TB through enhanced nursing interventions.

Research Questions

1. What are the roles of nurses in managing patients with drug-resistant tuberculosis in Yobe State?

2. What challenges do nurses face in the management of drug-resistant tuberculosis in Yobe State?

3. How can nursing practices be improved to enhance the management of drug-resistant tuberculosis in Yobe State?

Research Hypotheses

1. Nurses in Yobe State who receive regular training on the management of drug-resistant tuberculosis are more effective in delivering care than those who do not.

2. The lack of access to second-line drugs and diagnostic tools negatively affects nurses' ability to manage drug-resistant tuberculosis in Yobe State.

3. Patient education and adherence support by nurses significantly improve treatment outcomes for patients with drug-resistant tuberculosis in Yobe State.

Definitions of Terms

• Drug-Resistant Tuberculosis (DR-TB): A form of tuberculosis caused by strains of the bacteria that are resistant to at least one of the primary drugs used in tuberculosis treatment.

• Nursing Interventions in TB Management: Actions taken by nurses to provide care to TB patients, including administering medications, educating patients, and monitoring for side effects.

• Second-Line Drugs: Medications used to treat tuberculosis when the first-line drugs are ineffective due to resistance.
